By purchasing a ticket from Heart and Stroke Foundation of Canada (the “Foundation” or “Licensee”) for the 2020/21 Heart & Stroke Lottery, the Purchaser acknowledges they have read, understood, and agreed to these rules and regulations (“Rules”). “Purchaser” means a person: (i) purchasing a ticket(s) for his/her personal use; (ii) purchasing a ticket(s) on behalf of a group where the purchaser’s name and up to two additional names and/or a group name appear on all ticket(s), or (iii) purchasing a ticket that is allocated to a group of which the Purchaser is not a member. The Purchaser’s name shall always appear as the first name on any ticket purchased. All Purchasers are subject to these rules. The Foundation has no obligation or liability to any person named on a ticket other than the Purchaser.

All inquiries regarding the 1 to 2 odds of winning, conduct, prizing, and procedures of the Lottery are to be made to the Licensee prior to the order of ticket(s).

Eligibility

Ticket Purchasers and any other persons named on a ticket must be at least 18 years of age. Tickets must be purchased within the province of Ontario and cannot be mailed outside of Ontario. Only tickets that have been paid and verified as paid by the respective sales deadline are eligible. The Rules prohibit the following individuals and all persons residing in the same household as these individuals from purchasing a ticket: Employees and Board Directors of the Foundation; partners and employees of the ticket fulfillment service provider and its affiliates, and employees of licensed gaming suppliers for the Classic Lottery.

First, the definition of Lottery Process has been amended so that the Lottery Rules will remain in effect until Ontario Regulation 468/18 is amended, anticipated in July 2020. Second, an exception to Rule 2 has been added (refer to Rule 2c). Preface Regarding the First Lottery Process. The Canadian lottery is one of the most extensive and profitable lotteries in the world. All of the Canadian provinces are involved in lottery activities and lottery players enjoy a wide range of options, ranging from the interprovincial Lottery 6/49 which includes players in all provinces to the provincial 649 and Lotto Max lotteries along with a number of scratch card, poker and other. Taxes – Canada: Canadians don’t have to pay tax on their U.S. Lottery winnings in Canada, as lottery winnings are exempt from taxation under section 40 (2) (f) of the Income Tax Act of Canada.

Municipal Licence. A municipality may issue a licence to eligible local charitable or religious organizations for paper raffle lotteries where the total value of the prizes to be awarded does not exceed $50,000. Municipal licence applications may be made with the.

Tickets

Tickets will be available for purchase until February 12, 2021 at 11:59 pm or while supplies last. There are 157,500 tickets printed (150,000 tickets will be sold and 7,500 to be awarded as prizes). Tickets are $100 individually, $250 for a 3-Pack, or $375 for a 5-pack. Ticket numbers will be issued in sequential order. The official ticket is a receipt of purchase. Ticket purchases are not tax deductible. Only one method of payment may be used per order. Tickets are only refundable prior to the draw date for which they are eligible. Purchases placed online or where an email address is provided will receive an order confirmation by email followed by an email containing the digital ticket number (s) (allow 3-4 business days for receipt of email with digital ticket numbers (s) at the time of purchase. When selected, physical ticket(s) will be mailed to the address provided at the time of the order (allow a minimum of 14 business days). It is the Purchaser’s responsibility to ensure that ticket(s) are received before the applicable draws commence. The Foundation is not responsible for orders that are not received on time or at all. Notwithstanding anything contained herein to the contrary, the total liability of the Foundation to any Purchaser shall be limited to the purchase price of the ticket.

Up to three names may be recorded on a Classic Lottery ticket. The Purchaser must be named on the ticket. Any requests to change the name(s) on an official ticket must be received prior to the draw date for which the ticket is eligible. For mail orders only, the change of name must be accompanied with a copy of the Purchaser’s government issued photo ID.

Prizes

All values are in $CDN. There are 64,633 prizes to be won, valued at $5,155,204.24 (including all taxes, fees and freight). All prizes are tax-exempt for Ontario residents and must be accepted as awarded. No substitutions may be requested except where a cash alternative option is specified. Prizes awarded may not be as illustrated. Prizes may be subject to substitution of a prize of equivalent value and similar nature to be determined at the sole discretion of the Foundation.

Loyalty Prizes: There are five Loyalty prizes to be awarded, each consisting four ( 4) LG Appliances with a value of $7,966.45 each. Each of the 4 LG Appliance packages consists of: LG 6.3 cu. ft. Electric Slide-in Range with ProBake Convection™ and EasyClean® (valued at $2,485.99), LG 2.0 cu.ft. Over-the-Range Microwave with EasyClean® Interior. (valued at $677.99), LG Smudge Resistant Top Control Dishwasher with QuadWash®, WiFi Connectivity and 3rd Rack (valued at $1,299.49), LG 36“ French Door Refrigerator, 28 cu.ft. (valued at $3,502.99) VIP Prizes: There are two VIP prizes to be awarded, consisting of two prizes of $25,000 cash each. Super Bonus Prizes: There are 7,501 Super Bonus prizes, consisting of 1 prize of $50,000 cash, and 7,500 prizes of one free $100 Heart & Stroke Classic Lottery ticket. Early Bird Prize: There will be one Vehicle Package prize. This package includes a 2020 Lexus LC 500 ARTISAN SPIRIT CARBON FIBER (value at $136,383.88 ) and a 2020 Audi Q5 (value at $79,999.00). Total package value $216,382.88.

Grand Prizes: There are six Grand Prizes. Grand Prizes 1 & 2: Cash prize of $1 million each; Grand Prize 3: 2020 Ford F1500 King Ranch (valued at $84,974.87) plus Boat Package consisting of Legend Q-Series Cottage Pontoon Boat, Mercury 90 ELPT CT 4S Boat Motor, Shoreland’r PT2127AB Boat Trailer (valued at $59,899.85) or cash out equal to $140,000; Grand Prize 4: Winner must choose one of the two options available: option 1, a 2020 BMW X7 xDrive 40i (valued at $117,000) or option 2 2020 Audi Q8 55 (valued at $104,999.99). Draws

All prize draws for the Heart & Stroke Classic Lottery will take place at the MNP Toronto Office located at 111 Richmond Street West, Suite 300, Toronto, ON, M5H 2G4. Prizes will be drawn using Random Number Generator (RNG) software, and winning tickets will be eligible for subsequent draws. The sequence of draws will be as follows: The first draw will be for the Loyalty Prizes. Only tickets purchased by 11:59pm, October 16, 2020 will qualify for the Loyalty Prize Draws. The draws (5 prizes) will take place on October 29, 2020 at approx. 10:00am. The second draw will be for the VIP Prizes. Only tickets purchased by 11:59pm November 13, 2020 will qualify for the VIP Prize draws. The draws (2 prizes) will take place on November 26, 2020 at approx. 10:00am. The third draw will be for the Super Bonus Prizes. Only tickets purchased by 11:59pm, January 15, 2021 will qualify for the Super Bonus Prize draws. The draws (7,501 prizes) will take place on January 28, 2021 at approx.10:00am. The fourth draw will be for the Early Bird Prize. Only tickets purchased by 11:59pm, February 5, 2021 will qualify for the Early Bird Prize Draw. The draw (1 prize) will take place on February 18, 2021 at approx.10:00am. The last and final prize draw will be for the 50/50 Jackpot (see below), electronics, Visa Prepaid cards and 6 Grand Prizes. Only tickets purchased by 11:59pm, February 12, 2021 will qualify for the Final Prize Draw (57,118 prizes). The draws will take place on February 25, 2021 approx.10:30am.*

* If all tickets for the Classic Lottery are sold before the March Money Deadline (December 11, 2020 at 11:59pm), the draw for the Super Bonus Prize will be made on December 21, 2020 at approx. 10:15am.

** If all tickets for the Classic Lottery are sold before the March Money Deadline (December 11, 2020 at 11:59pm), draws for all remaining prizes will be made on January 7, 2021 at approx. 10:00am. The first draw will be for the Early Bird Prize; followed by the electronics, Visa Prepaid cards and the 6 Grand Prizes.

*** If all tickets for the Classic Lottery are sold before the March Money Deadline (December 11, 2020 at 11:59pm), the winners announcements will take place on January 8, 2021 at approx. 4pm.

Prize Winner Notification

All winners (in each case, the Purchaser named on the drawn ticket) will be notified in writing within 4 – 6 weeks of the prize draw and major prize winners (greater than $10,000) may also be contacted by phone. From the first draw date on October 29, 2020, winners will be posted on the Heart & Stroke Lottery website and social media pages. A complete searchable winners list will be available at www.heartandstrokelottery.ca/winners at approx.4pm on February 26, 2021. . If all tickets for the Classic Lottery are sold before the March Money Deadline (December 11, 2020 at 11:59pm), the winners list will be available at www.heartandstrokelottery.ca/winners at approx. 4pm on January 8, 2021. A copy of a government issued photo ID may be required to claim prizes greater than $10,000. Prize(s) must be claimed no later than six months after the final draw date. Unclaimed prizes will be reported and a charitable allocation to the Foundation will be approved by the Alcohol and Gaming Commission of Ontario (“AGCO”).

Prize Claim

Prizes will be awarded to the Purchaser named on the official ticket. By claiming a prize, the winner consents to the use of their likeness (photograph, if available), first and last name, ticket number, city or town of residence, video recording (when available only) and transcription/recording of the winner notification phone call to be used for any publicity purposes by the Foundation. Please allow a minimum of 4 weeks for delivery of prize from draw date. Delivery time for vehicles and boats may be longer. Vehicle and boat winners are responsible to licence and insure the vehicle or boat from the time of ownership.

The Foundation is only responsible for communicating with the Purchasers and is not responsible for any prize entitlement of any other individual(s). Prizes stated are retail value as of date contracts were agreed with suppliers and are independently verified by prize supplier, as per AGCO rules.

Privacy

We are committed to protecting the privacy of your personal information. We will use your personal information to administer and evaluate the Lottery, including contacting, announcing and promoting Prize winners. In addition, we may use this information to send you donor-related communications and for tax receipting purposes, where required. Occasionally, we may contact you with missionrelated or program-related communications. 50/50 Draw

State Lottery Rules

All Rules applicable to the Lottery also apply to the 50/50 Draw. 50/50 Draw tickets must be ordered in conjunction with a Classic Lottery ticket. The winner of the 50/50 Draw prize will receive 50% of the total accumulated jackpot. There is a minimum guaranteed payout of $500,000. The total jackpot is dependent upon the gross revenue generated from tickets sold. Tickets are a 5-Pack for $25, 15-Pack for $50 or 25-Pack for $75. The total number of 50/50 tickets available for sale is unlimited.

Purchases placed online or where an email address is provided will receive an order confirmation by email followed by an email containing the digital ticket number (s) (allow 3 to 4 business days for receipt of email with digital ticket numbers). Purchasers without an email address will have the option to request physical ticket(s) at the time of purchase. Gifting

Purchaser(s) will be notified of winnings. It is the sole responsibility of the Purchaser to track distribution of ticket number(s) and allocate any winnings. Cheques will be made payable to the Purchaser. Cheques will be mailed to the address provided by the Purchaser at the time of ticket order.

Prizes

All values are in $CDN. There are 3,177 prizes to be won for a total value of $1,005,000 (including all applicable fees and taxes). All prizes are tax-exempt for Ontario residents, and must be accepted as awarded. No substitutions except where a cash alternative option is specified. Prizes awarded may not be as illustrated.

Bonus Draw: There will be 15 winners of $5,000 each to be awarded. Daily Cash Prizes: There are a total of 62 prizes of $10,000 each to be awarded for daily draws. Daily Visa Prepaid Cards: There are a total of 3,100 $100 Visa Prepaid Cards to be awarded. There will be 100 Prepaid Visa Prepaid Cards awarded daily between March 1 to 31, 2021.
